It was truly a pleasure to meet you last week
at the RAI course in OKC. We have always enjoyed our working relationship with
the members of the CAA and look forward to working with you in the future.
We have an internal requirement to capture as
much factual, releasable data related to events involving our products as
possible and although I recognize we do not have any direct participation or
involvement with the subject investigation, we would appreciate any factual
information that you can provide. This information will be placed in our
secured Safety Management database and will not be disseminated to others
either within or outside the company.
We understood from the preliminary report was
aircraft originated in Bucaramanga and was enroute to Santander transporting
cargo when it contacted wires. Any additional information regarding history of
flight, aircraft and engine damage, engine serial number, weather, etc would be
appreciated. Again, it was a pleasure to meet you.

The Captain of COA 881 reported the following:
"Takeoff was planned/scheduled at MGTW. As
loading was completed there was no room for additional fuel. While at the gate
we got word that our departure fix was expecting to have delays or be stopped
for WX. We departed the gate at a GWT of
154,800 with 35,200# fuel expecting a reasonable taxi time to takeoff. We were
expecting a quick departure, but experienced incremental delays which used
valuable fuel. Finally, at takeoff we had 34,300# fuel. Min fuel was 33,800#.
En route and arrival WX looked good and no deviations were anticipated because
of weather en route. When we got to BOG approach control they began vectors to
sequence us with two or three

Aviation Safety Seminar we presented our work and examples of the worksheets

after reading the academic paper would be most appreciated. Our effort is to provide
safety officers a way to how senior management the cost benefits of some safety
programs. The worksheets allow for customized Inputs of cost categories. We
then calculate the organizational costs which is useful to senior management. This
method has been well received.
Further
discussion on how we can help your operation would be of interest to us.
You
mentioned your involvement with CRM. We provide a complete Safety Management
System program using MITRE courseware. As you know, CRM Is part of a successful
SMS. We would like to develop the opportunity to expand your SMS with our world
recognized courseware and customized approach.
